Bank Statement CSV Template and Example Rows
Download a simple bank statement CSV template with fictional rows and learn how to format dates, descriptions, debits, credits, and balances.

A consistent CSV structure makes statement review easier. This template has Date, Description, Debit, Credit, and Balance columns with fictional examples. Download it to see the layout, then replace every sample row with your own checked transactions.
Five-column example
| Date | Description | Debit | Credit | Balance |
|---|---|---|---|---|
| 2026-10-02 | Office supplies | 24.50 | 975.50 | |
| 2026-10-04 | Client transfer | 250.00 | 1225.50 |
Use a consistent date format such as YYYY-MM-DD. Put a withdrawal in Debit and a deposit in Credit, leaving the other field blank. Keep currency symbols out of numeric cells. If your statement does not show a running balance, leave Balance blank rather than calculating or inventing one.
Before importing the template anywhere
- Delete the fictional sample rows.
- Compare your row count and date range with the source statement.
- Check whether the receiving software wants a single signed Amount column instead of Debit and Credit.
- Save the file as UTF-8 CSV if descriptions include non-English characters.
- Keep your original statement and a copy of the final CSV for reconciliation.
QuickBooks Online accepts a three-column Date, Description, Amount layout or a four-column layout with separate Credit and Debit fields. Its column mapping and sign conventions may differ from this five-column review template, so follow the import preview rather than uploading this file unchanged.
